If you're reading this, you're probably in the middle of one of the hardest decisions a family makes. We built CareWitness to give you a clear, verifiable picture of every licensed senior care facility in your state — with every fact tied back to the public record it came from.
Built on public records
Two sources build every facility page: state licensing registries (the source of record for what exists) and CMS Care Compare (quality data on nursing facilities). The methodology walks through exactly how each layer is collected, cross-checked, and refreshed.
Quality data, front and center
When CMS flags a facility with an abuse icon or Special Focus designation, it appears prominently on that facility's page. Fines are shown with both the count and the total dollar amount. RN turnover is benchmarked against the state average. Every one of these signals is already documented by the federal government — a family shouldn't have to dig through CMS Care Compare to find them.
Every licensed facility in the state
Starting from a state's own licensing registry means every facility the state has approved to operate appears on CareWitness. In Texas alone, that's 3,588 facilities: 2,011 assisted living residences, 1,176 nursing facilities, 395 adult day care centers, and 6 hospital-based nursing facilities. State-licensed-only facilities — the ones without federal CMS certification — are just as visible as the major chains.
What we stand behind
- State licensing registries are the source of record for what facilities exist. CMS Care Compare is the source for quality data on nursing facilities. Anything else is supplementary, labeled, and dated.
- CMS quality data appears on every facility page that has it — abuse flags, fines, Special Focus designations, all of it — unmodified from the source.
- Every listing is a real, state-licensed facility.
- Every fact traces to a public record, with the date it was last verified visible on the page.
Where we are now
CareWitness currently covers Texas, with state-by-state expansion underway. Every facility listing shows the snapshot date of each data source it uses.
