About CareWitness

Finding senior care is one of the hardest decisions a family makes. Most directories make it harder — burying the data that matters, defaulting to whoever paid for the top spot, and treating every family the same.

CareWitness exists to give families a clearer picture. We pull facility ratings, inspection records, and penalty histories directly from the Centers for Medicare & Medicaid Services (CMS) and present them without modification. CMS data is never adjusted, filtered, or hidden — regardless of whether a facility has a presence on our platform.

We also publish information most directories ignore: languages spoken by staff, cultural programming, dietary accommodations, and religious affiliations. Senior care should work for every family, not just those whose needs fit a default.

Facilities can claim their listing to keep their information current and stand out to families who are a good fit. CMS inspection data and ratings remain unchanged for all facilities, claimed or not.

CareWitness currently covers the greater Houston metro area. We update our data quarterly as new CMS records are released.
